What you will do

• Provide professional nursing care across ambulatory, telephone triage, and telehealth settings.
• Assess, plan, implement, and evaluate patient care using the nursing process.
• Collaborate with physicians, advanced practice providers, and interdisciplinary teams on care plans.
• Administer medications, vaccines, and treatments, and perform point-of-care testing within scope of practice.
• Educate patients on self-management, medications, nutrition, health, and wellness.
• Document care in the EMR, support pre-visit planning, respond to urgent situations, and mentor clinical staff.

Requirements

• Associate of Science in Nursing required; Bachelor of Science in Nursing preferred.
• Current Registered Nurse license in New Hampshire or an NLC state/Massachusetts required.
• Basic Life Support certification required.
• At least one year of professional nursing experience preferred; ambulatory care experience is an advantage.
• Knowledge of nursing principles, EMR systems, and patient-centered care models.
• Strong communication, time management, adaptability, teamwork, and attention to detail.
