Clinical Adherence Nurse (RN) (Bilingual)

Location: Fully remote, but candidates must reside legally in Puerto Rico and maintain Puerto Rico residency throughout employment. Work authorization in Puerto Rico and the United States is required; visa sponsorship is not available. Evening and weekend availability is required. Available schedules are Saturday–Wednesday or Sunday–Thursday, with shifts from 12:00 p.m. to 8:00 p.m., 1:00 p.m. to 9:00 p.m., 2:00 p.m. to 10:00 p.m., or 3:00 p.m. to 11:00 p.m. EST.

What you will do

• Engage and enroll patients through video messaging, video consultations, phone calls, and text communication.
• Provide personalized medication education, motivational coaching, case management, and chronic disease support.
• Conduct medication reconciliation consultations and review patient-submitted medication videos for correct administration, adherence, and potential side effects.
• Assess medical, behavioral, and social needs, escalate cases requiring clinical intervention, and connect patients with providers and community resources.
• Support patients with inhaler technique, medication safety, digital platform troubleshooting, and long-term adherence strategies.
• Maintain accurate documentation and collaborate with providers, pharmacies, health plans, and cross-functional teams to improve clinical content and protocols.

Requirements

• Legal residency in Puerto Rico is mandatory and must be maintained throughout employment.
• Registered Nurse license in good standing with at least five years of nursing experience.
• At least five years of experience in patient engagement, health education, care coordination, or a related field.
• Fluency in both English and Spanish is required, including reading, writing, speaking, and comprehension.
• Experience with chronic disease management, medication adherence, patient coaching, social determinants of health, and resource coordination.
• Proficiency with EHRs, mobile applications, digital health platforms, and patient communication by phone, video, and chat.

Nice to have

• Experience in video communication, telehealth, community health, or digital health.
• Health coaching certification or willingness to obtain one.
• Knowledge of Directly Observed Therapy models and experience with Medicaid and Medicare dual-eligible populations.
• Familiarity with Salesforce or other CRM platforms.
