What you will do
- Manage and prioritize individual members on their journey to metabolic reversal and self-care management.
- Deliver remote coaching via apps, video conferencing, and electronic health systems.
- Conduct daily monitoring of health data, including weight, continuous blood glucose (CGM), blood pressure, sleep, and breathing.
- Provide feedback on daily metrics to ensure program adherence and goal achievement.
- Collaborate with a multidisciplinary team of providers, registered nurses, and operations teams.
- Maintain participant records in a secure and compliant fashion.
Requirements
- 3+ years of experience in health/nutrition counseling, health coaching, fitness, or life coaching.
- Active license in the US; Health Coaching or Health Educator certifications are preferred.
- Bilingual fluency in both English and Spanish.
- Proficiency in motivational interviewing and driving behavior change.
- Comfort integrating technical data outputs into patient care and working in high-growth environments.
- Must be based in the USA.
