Compensation: $27.00 - $30.00 per hour

What you will do

• Lead patient registration, enrollment, and community engagement through outreach and outbound calls.
• Provide comprehensive care navigation, including appointment logistics, prescription drug support, and referral coordination.
• Assess social determinants of health (SDOH) needs and enroll patients in appropriate support programs.
• Collaborate with a multidisciplinary care team to develop care plans and solve complex patient needs.
• Organize and participate in community events to build trust and provide health education.
• Document all patient interactions across various internal systems and EHR tools.

Requirements

• Fluency (verbal and written) in English and Spanish.
• Must be authorized to work in the United States.
• CHW certification OR demonstration of 2,000+ hours of CHW work in the past three years.
• 1-3 years of healthcare experience and 2-5 years of community advocacy or organizing experience.
• Fully vaccinated against COVID-19.
• Reliable transportation for travel to community events and patient appointments.

Nice to have

• Familiarity with Google Workspace and experience documenting in an EHR.
• Training in motivational interviewing.
• Strong ties to the local Latino community and knowledge of local SDOH resources.
