Team Lead, Care Management Support Coordinator (Healthcare): Supporting care management operations through member outreach, inbound call handling, service scheduling, record documentation, and weekly data reporting with an accent on Medicaid LTSS services, escalations, and stakeholder communication. Focus on coordinating care resources, presenting findings, maintaining regulatory records, and building Excel reports with pivot tables and conditional formatting.
Location: Remote in Pennsylvania, United States. Work schedule: Monday–Friday, 8:00 AM–5:00 PM EST.
Salary: $20.39–$34.71 per hour.

What you will do
- Conduct member outreach by phone regarding care plan next steps, scheduling, resources, questions, and ongoing education.
- Serve as frontline support for member and provider inquiries, requests, and concerns.
- Coordinate member onboarding, welcome communications, educational materials, and connections to health plan and community resources.
- Document and maintain non-clinical member records in accordance with state and regulatory requirements.
- Escalate requests and inquiries to management and support training for new hires.
- Assist People Leaders with weekly data reporting for up to 50% of the role.
Requirements
- High school diploma or GED required.
- 2–4 years of related experience; transferable experience includes 4+ years processing healthcare or insurance authorizations and coordinating Medicaid Long-Term Services and Supports.
- Strong problem-solving, communication, organization, and time-management skills.
- Ability to adapt to changing business needs, work under pressure, and handle escalations.
- Moderately strong Excel skills, including data extraction, pivot tables, and conditional formatting, or willingness to develop these skills with generative AI tools.
- Ability to present findings clearly and concisely to People Leaders, peers, and other stakeholders.
