Clinical Documentation Specialist (Remote) (Healthcare Documentation and Coding): Reviewing inpatient medical records and improving the completeness, accuracy, and integrity of clinical documentation with an accent on severity of illness, risk of mortality, quality indicators, and level of services. Focus on conducting concurrent reviews, educating physicians and healthcare staff, and formulating compliant documentation queries.
Location: Remote position associated with Davenport, Clinton, Maywood, Newton, Waterloo, Des Moines, and Brighton locations in the United States.

What you will do
- Review selected inpatient health records concurrently for legibility, clarity, completeness, consistency, and precision.
- Support the integrity of clinical documentation so that severity of illness, risk of mortality, quality indicators, and service levels are accurately reflected.
- Educate physicians and healthcare team members on clinical documentation requirements.
- Monitor provider engagement and collaborate with healthcare professionals.
- Formulate compliant documentation clarifications and queries according to established procedures.
Requirements
- Associate or diploma degree in Health Information Technology, or an advanced nursing, Nurse Practitioner, Advanced Practice Nurse, or Physician Assistant degree.
- Without a college degree, three years of experience as an inpatient coder or clinical documentation specialist is required.
- Clinical or coding experience in critical care, medical or surgical inpatient care is preferred.
- Strong communication, collaboration, relationship-building, critical-thinking, prioritization, and organizational skills.
- Ability to use standardized desktop and Windows-based systems, with data entry and typing skills of at least 30 words per minute.
Nice to have
- RN, RHIA, RHIT, CCS, CCDS, or CDIP certification.
