RN - Certified Diabetes Educator (Diabetes Education)

Location: Diabetes and Endocrine Care, 9 Loon Hill Rd Suite 301, Dracut, MA 01826, at Lowell General Hospital - Dracut Urgent Care

Pay range: $40.53–$51.67 per hour, based on full-time equivalent.

What you will do

• Provide diabetes self-management education, health teaching, and counseling to patients and their families.
• Coordinate family-centered, multidisciplinary care for patients with Type 1, Type 2, steroid-induced, and cystic-fibrosis-related diabetes.
• Conduct individual and group patient visits, lifestyle coaching, clinical assessments, and care-plan evaluations.
• Manage cases by interpreting laboratory results, ordering therapeutics and durable medical supplies, renewing prescriptions, and coordinating referrals and diagnostic studies.
• Develop and document individualized education and discharge plans using appropriate community resources.
• Support clinical quality improvement, evidence-based practice, staff development, precepting, and departmental committees.

Requirements

• Massachusetts RN licensure required.
• Three years of RN experience.
• Current Basic Life Support certification.
• Certification in Diabetes Education (CDE), or the ability to obtain it within one year of hire.
• Ability to communicate effectively with patients, families, physicians, and the healthcare team.
• Ability to perform prolonged standing and walking, occasional lifting of 30–35 lbs, and patient maneuvering.

Nice to have

• Bachelor of Science in Nursing.
