What you will do
- Conduct comprehensive initial and ongoing evaluations of residents’ medical and psychosocial needs.
- Diagnose and treat acute and chronic conditions common among older adults.
- Develop individualized care plans with nursing staff, therapists, and other healthcare professionals.
- Manage residents’ medications and document medical records accurately.
- Educate and support family members and caregivers.
- Participate in interdisciplinary care conferences and quality improvement initiatives.
Requirements
- Board-certified or board-eligible in Geriatric Medicine or Internal Medicine.
- Valid medical license in New Hampshire, or active progress toward obtaining one.
- At least two years of experience in a geriatric setting, such as a nursing home or long-term care facility.
- Excellent communication, collaboration, and interpersonal skills.
- Compassionate and patient approach when working with older adults.
- Proficiency with EPIC EMR.
