RN - Certified Diabetes Educator (Diabetes Education): Providing diabetes self-management education, nursing care, and individualized discharge planning for patients and families with an accent on complex illness, clinical assessment, and multidisciplinary coordination. Focus on developing patient care and education plans, managing diabetes-related referrals and treatments, and improving clinical outcomes through evidence-based practice.
Location: Lowell General Hospital - Diabetes and Endocrine, Dracut, Massachusetts, United States. Full-time schedule, Monday–Friday, 8:00 a.m.–5:00 p.m., 40 hours per week.
Pay range: $40.53–$51.67 based on full-time equivalent.

What you will do
- Provide diabetes self-management education, lifestyle coaching, counseling, and individual and group visits for patients and families.
- Coordinate a family-centered, multidisciplinary program for patients with Type 1, Type 2, steroid-induced, and cystic-fibrosis-related diabetes.
- Conduct clinical assessments and develop, implement, evaluate, and revise individualized care, education, and discharge plans.
- Provide case management, interpret laboratory results, coordinate referrals and diagnostic studies, and order therapeutics, supplies, and prescription refills as appropriate.
- Serve as an expert clinician and diabetes educator while integrating evidence-based practice and research into patient care.
- Support staff development through precepting, coaching, professional development, and participation in departmental committees.
Requirements
- Massachusetts RN license required.
- Three years of RN experience required.
- Current Basic Life Support certification required.
- Certification in Diabetes Education required, or must be obtained within one year of hire.
- Ability to communicate effectively with patients, families, physicians, and the healthcare team.
- Ability to apply the nursing process and perform physical duties including prolonged standing and walking, patient lifting and turning, and exposure to blood-borne pathogens and infectious disease.
