LCSW LPC LMFT Behavioral Health Counselor Bilingual Spanish (Behavioral Health)

Location: Onsite in the office in Conklin, New York

Salary: $67,100–$111,800 USD per year, depending on experience and geographic location; eligible for an annual bonus plan.

What you will do

• Conduct individual, group, and family counseling appointments for clients seeking mental health support.
• Screen and assess patients for mental health and substance use disorders using evidence-based tools.
• Implement evidence-based treatments, monitor progress, and provide ongoing clinical support.
• Educate patients and families about disorders, treatment options, and participation in care.
• Coordinate referrals with providers, community resources, and outside agencies.
• Coordinate care after emergency department presentations or inpatient admissions with providers and population health teams.

Requirements

• LCSW, LPC, or LMFT qualification with an independent license.
• Current, unrestricted New York license required.
• At least two years of therapy experience.
• Bilingual Spanish required.
• Experience with population management.
• Ability to work in the Conklin, New York office required.
