Salary: $124,800.00–$155,933.24 annually, with sign-on bonus eligibility

What you will do
- Provide comprehensive pharmacy services for patients with advanced heart failure, including heart transplant recipients and LVAD patients.
- Review and approve medication orders and collaborate with inpatient pharmacists, physicians, surgeons, and multidisciplinary heart failure teams.
- Complete pre-transplant evaluations, post-operative medication reconciliation, discharge reconciliation, and patient education.
- Deliver heart transplant medication discharge teaching and LVAD warfarin/enoxaparin education in compliance with CMS regulations.
- Facilitate transitions of care, medication access, outpatient cardiology support, and maintenance of institutional guidelines and protocols.
- Participate in quality, transplant listing, LVAD, and morbidity and mortality meetings while precepting pharmacy residents and students.
Requirements
- PharmD and two years of hospital clinical pharmacy experience, or a bachelor's degree in pharmacy and five years of hospital clinical pharmacy experience.
- Active pharmacist license.
- Knowledge of clinical best practices, medication optimization, formularies, chronic condition management, and population management.
- Strong verbal, written, presentation, communication, organizational, problem-solving, and project management skills.
- Ability to work independently and collaboratively, prioritize critical issues, protect patient confidentiality, and use electronic medical records and healthcare collaboration technologies.
- Ability to stand and walk for 25–75% of the day, occasionally lift up to 30 pounds, and work around hazardous chemicals and broken glass.
Nice to have
- Three years of hospital clinical pharmacy experience with a PharmD, or seven years with a bachelor's degree in pharmacy.
- PGY1 pharmacy residency and PGY2 residency in a relevant practice area.
- Pharmacy board certification or an active related BPS credential.
- Experience in drug utilization review and analysis.
