Registered Nurse - 1099 Contract $120 per visit (Hartford & New Haven, CT / Field-based)

Location: Field-based in Connecticut, covering West Hartford, New Haven, Bridgeport, Waterbury, and New Britain

Compensation: $120 per field visit

What you will do

• Conduct in-home clinical assessments and help patients and caregivers begin services successfully.
• Develop, implement, and update individualized plans of care.
• Build relationships with patients and caregivers through home visits and clinical follow-up.
• Identify changes in condition, safety concerns, and barriers to care using clinical judgment.
• Coordinate care with physicians, caregivers, clinical teams, and community resources.
• Educate patients and caregivers, review documentation, advocate for patients, and maintain accurate clinical records.

Requirements

• Graduate of an accredited nursing program.
• Active, unrestricted Registered Nurse license in Connecticut.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently and communicate with patients, families, caregivers, physicians, and care teams.
• Ability to travel by car within the assigned geographic area, with a valid driver's license and active auto insurance.
• Comfort using clinical, case management, and productivity software.

Nice to have

• Experience in home care, community health, case management, or care coordination.
