1099 Care Manager, Health Homes (Healthcare)

Location: Field-based in Brooklyn, the Bronx, Manhattan, and Queens, New York City; frequent travel to patients' homes is required.

Compensation: $50 per completed field visit.

What you will do

• Outreach to potentially eligible patients and obtain consent for enrollment in the Health Home Care Management Program.
• Conduct eligibility screenings, initial assessments, and reassessments covering medical, mental health, substance use, financial, housing, and support needs.
• Develop, implement, and coordinate patient-centered care plans for patients with chronic diseases and document outcomes.
• Support patients through referrals, access resolution, local resources, and relationships with healthcare providers.
• Coordinate services with pharmacies, insurers, hospitals, family caregivers, and other providers.
• Conduct home visits, provide crisis intervention when needed, and maintain compliant documentation and confidentiality.

Requirements

• Associate's degree required.
• At least 2 years of care management experience.
• Ability to travel in the field, visit patients in person, and cover at least Brooklyn and Queens.
• Knowledge of local community resources and experience with data reporting, documentation, and outcomes.
• Strong communication, assessment, empathy, and crisis-management skills.
• Ability to manage a high-volume caseload and use multiple data management systems, such as Salesforce.
