What you will do
- Provide professional nursing care to homebound and terminally ill patients during variable shifts.
- Review records, complete OASIS assessments, develop care and discharge plans, and coordinate referrals and case conferences.
- Perform cardiovascular, pulmonary, neurological, gastrointestinal, skin, pain, intake/output, and other patient assessments.
- Provide enteral feeding, IV, PICC, central-line, tracheostomy, colostomy, suctioning, and medication administration procedures.
- Educate patients and caregivers, promote independence, communicate changes and orders to physicians, and complete required documentation.
- Supervise LPNs and home health aides, participate in admissions, discharges, on-call procedures, and quality improvement activities.
Requirements
- Graduation from an accredited school of nursing.
- At least one year of professional hospital experience or prior home health experience.
- Active Tennessee RN license required.
- CPR certification, a valid Tennessee Class D driver’s license, and state-required minimum insurance coverage required.
- Driving record must meet Covenant Health standards.
- Ability to work two assigned 12-hour shifts starting at 4 p.m. and two required 12-hour on-call shifts starting at 4 a.m. on assigned workdays.
