Salary: $24.65–$30.82 per hour

What you will do
- Support transitions across the care continuum through telephonic outreach, referrals, appointment scheduling, and coordination with providers, hospital staff, community agencies, and affiliated organizations.
- Conduct post-discharge outreach to confirm follow-up appointments, prescriptions, homecare services, and early symptom management.
- Create and distribute patient activity, admission, discharge, walk-around, and other clinical operations reports.
- Review referrals and hospital admissions, track level-of-care determinations, and identify authorization, denial, pending, and covered-day information.
- Monitor emergency department utilization and communicate information to clinical staff for timely intervention.
- Enroll patients, maintain registries and spreadsheets, and collaborate with the Resource Team and clinical operations staff.
Requirements
- High school diploma or GED.
- At least two years of experience in a clinical setting, such as a nursing unit, care management, or physician’s office.
- Experience with electronic medical records and general computer applications.
- Strong proficiency with Microsoft Office, Word, and Excel.
- Customer-focused interpersonal, communication, organizational, prioritization, and problem-solving skills.
- Ability to work at the Tufts Medical Center clinical site.
Nice to have
- College degree.
- Medical Assistant certification.
- Three years of related experience.
