What you will do
- Coordinate multidisciplinary care across the healthcare continuum and support smooth transitions from hospital to home.
- Serve as a liaison, advocate, and educator for patients, providers, clinicians, and community resources.
- Triage new patient referrals and facilitate timely access to care.
- Conduct post-discharge follow-up calls and help patients navigate barriers to treatment.
- Assess patient needs, integrate them into care plans, and encourage shared treatment decisions.
- Collect and report information about barriers, hospital experiences, medications, and prior treatments.
Requirements
- Massachusetts RN licensure required.
- At least three years of RN experience.
- Current Basic Life Support (BLS) certification.
- Ability to lift and maneuver patients, with occasional lifting of 30–35 lbs.
- Ability to work prolonged periods standing and walking and to manage exposure to infectious diseases, blood-borne pathogens, and possible carcinogenic chemicals.

- Bachelor of Science in Nursing (BSN).
