Manager, Care Coaching - WI Rapids

Location: Remote from home; must reside within the State of Wisconsin. The role supports staff in Wisconsin Rapids and surrounding areas, with travel at least once per month and possible additional travel to offices, meetings, and quality visits. Working hours are Monday–Friday, 8:00 am–4:30 pm CST.

Salary: $78,400–$107,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Provide daily leadership, mentoring, and coaching to Field Nurses, Care Coaches, and IDT staff.
• Identify learning needs, set expectations, and coordinate orientation and ongoing training.
• Assess staff competencies and performance standards and review related documentation.
• Evaluate utilization and the quality of care management delivered to members.
• Collaborate across departments and educate community partners to support effective provider services.
• Participate in hiring, selection, onboarding, training, in-person unit meetings, and quality visits.

Requirements

• Bachelor’s degree in human services or a related field, or an associate degree in nursing.
• At least 1 year of leadership experience in human services.
• At least 3 years of experience working in the Family Care program.
• Must reside within the State of Wisconsin.
• Experience supporting individuals with intellectual or developmental disabilities, physical disabilities, or frail elders is preferred.
• Ability to work from a dedicated space with internet speeds of at least 25 Mbps download and 10 Mbps upload while protecting member PHI and HIPAA information.

Nice to have

• Residence within 2 hours of Wisconsin Rapids.
• Five years of experience working with individuals with intellectual or developmental disabilities, physical disabilities, or frail elders.
