RN-Onsite Care Coordinator Nurse (Healthcare)

Location: Onsite in La Crosse, Wisconsin, United States. Occasional or permanent work-from-home may be possible, subject to cable or fiber internet with at least 10 Mbps download and 5 Mbps upload.

Salary: $38–$64 per hour, depending on experience and geographic location. Eligible for an annual bonus plan.

What you will do

• Serve as an onsite clinical resource and healthcare advocate for employees and covered dependents.
• Coordinate care for members with complex, chronic, high-risk, or emerging healthcare needs.
• Guide members through benefits, provider selection, treatment options, and healthcare navigation.
• Identify care gaps and members who may benefit from clinical, wellness, behavioral health, and disease management programs.
• Conduct outreach, education, referrals, warm handoffs, and follow-up while tracking engagement outcomes.
• Collaborate with Kwik Trip leadership, Allegiance care management, healthcare providers, and clinical program vendors.

Requirements

• Current, unrestricted Registered Nurse license.
• At least three years of experience in care management, case management, population health, utilization management, clinical coordination, employee health, or a related healthcare setting.
• Strong understanding of care coordination, chronic disease management, behavioral health, and healthcare delivery systems.
• Strong communication, relationship-building, coaching, organization, documentation, and follow-through skills.
• Ability to work independently and collaborate across multidisciplinary teams.
• Onsite availability in La Crosse, Wisconsin, United States is required for the role.

Nice to have

• Certified Case Manager, Board Certified Case Manager, or similar certification.
• Experience with health plans, self-funded employer groups, employee health programs, population health initiatives, or onsite health services.
• Experience with complex case management, behavioral health coordination, specialty medication management, or chronic disease programs.
• Familiarity with claims-based outreach, healthcare analytics, vendor management, and program engagement strategies.
