Supportive Housing Case Manager

Location: Longview, Washington, USA. This is an onsite role requiring daily attendance in the office, community outreach, local travel, and occasional client home visits.

Salary: $21.50–$23.95 per hour.

What you will do

• Conduct client assessments and interviews to determine eligibility and develop action plans.
• Maintain case notes, client files, outcome tracking, and required data updates.
• Provide life skills development and targeted interventions for clients at risk of eviction.
• Connect clients with medical, mental health, counseling, and community support services.
• Guide clients through applications and re-determinations for SSI, SSDI, Medicaid, Medicare, SNAP, and other benefits.
• Travel throughout the area, provide transportation as needed, participate in meetings, and represent Compass in community settings.

Requirements

• Direct housing experience must be detailed in the resume.
• Bachelor's degree in social services or a related field plus one year of supportive housing or independent living coordination experience; or a high school diploma/GED plus two years of relevant experience.
• Valid driver's license, insurance, and access to a vehicle insured for the applicant as a driver.
• Ability to work a flexible schedule, including occasional nights and weekends, and to travel in the community.
• Ability to use Microsoft Office, Zoom, PCs, Android devices, laptops, cell phones, and designated software.
• Ability to perform CPR/First Aid as needed, lift up to 50 pounds, and meet the minimum age requirement of 21.
