Location: Must be based in or near Atlanta, Georgia and able to travel locally for in-home visits

Salary: $140,000 - $165,000 USD

What you will do

  • Provide longitudinal primary care directly in members' homes.
  • Manage a dedicated panel of patients with chronic, complex, and advanced medical needs.
  • Develop trust-based relationships with patients, caregivers, and families.
  • Create and execute personalized care plans to improve patient outcomes.
  • Collaborate with physicians and community providers using the Clover Assistant platform.
  • Participate in a shared on-call rotation to ensure continuous member support.

Requirements

  • Certified Physician Assistant (PA-C).
  • Based in or near Atlanta, Georgia.
  • At least 1 year of PA experience in a clinical setting.
  • Active DEA registration and current Basic Life Support (BLS) certification.
  • Experience managing chronic illness and medically complex patients.
  • Ability to work autonomously while collaborating with a broader care team.

Nice to have

  • Experience in home care, house calls, geriatrics, complex care, or value-based care.
