What you will do

• Manage an assigned caseload of members with complex behavioral health needs, conducting regular face-to-face visits.
• Provide relationship-based outreach, care coordination, and brief skills-based behavioral health support.
• Conduct home visits, community-based meetings, and field-based contacts as required by clinical needs.
• Facilitate warm handoffs and referrals to psychotherapy, psychiatry, and specialty mental health services.
• Collaborate with a multidisciplinary care team to develop and implement shared care plans.
• Document all interactions in compliance with health plan standards and track behavioral health outcome measures.

Requirements

• Master’s degree in Social Work required.
• Active, unrestricted Clinical Social Work License (LCSW) required.
• Fluency in English and Spanish (verbal and written) required.
• Must be authorized to work in the United States.
• 1–3 years of healthcare experience or community-based healthcare navigation.
• Reliable transportation for travel to community events and member appointments.

Nice to have

• Strong ties to the local Latino community and knowledge of Latino cultural norms.
• Experience with SMI/SUD populations.
• Training in motivational interviewing.
• Familiarity with EHR documentation and Google Workspace.
