Salary: $140,000 - $165,000 USD

What you will do

• Manage a dedicated patient panel and deliver ongoing primary care directly in members' homes.
• Provide clinical support for members with chronic illnesses and advanced medical needs.
• Develop and maintain longitudinal care plans to ensure continuity and quality of outcomes.
• Participate in a shared on-call rotation to ensure continuous patient support.
• Collaborate with providers and the In-Home Care (IHC) team to elevate community health.

Requirements

• Active and unencumbered NJ Registered Nurse license.
• Certified Advanced Practice Nurse Practitioner (APN).
• Master’s degree or post-master’s certificate in advanced clinical practice.
• Minimum of 1 year of NP experience in a clinical setting.
• Active DEA and CDS registration.
• Current Basic Life Support (BLS) certification.
