What you will do

• Partner with members and families to identify support needs and personal health goals.
• Collaborate with clinical and social support colleagues to address physical and behavioral health needs.
• Conduct thorough face-to-face visits to assess functional, psycho-social, and financial status.
• Authorize appropriate home and community-based services based on established criteria.
• Manage daily tasks and stakeholder requests while maintaining accurate member records.

Requirements

• Must live within Shelby County, Tennessee
• Active Registered Nurse license in Tennessee or NLC state, or Masters in Social Work with active unrestricted license (LCSW, LMSW, or LAPSW)
• Minimum 2 years of clinical experience
• Valid driver's license and ability to travel to member homes
• Proficiency in Microsoft Office and email
• Ability to work an 8:00am – 5:00pm EST schedule
