What you will do

• Partner with members and families to identify necessary supports for health, employment, and independent living.
• Collaborate with clinical and social support teams to address physical and behavioral health needs of each member.
• Conduct thorough face-to-face home visits and objective assessments to determine current status and needs.
• Utilize specific criteria to authorize and verify the delivery of appropriate home and community-based services.

Requirements

• Must reside in Shelby County, TN.
• 3 years of experience in a clinical setting.
• Active Registered Nurse license in Tennessee (or via Nurse Licensure Compact) OR Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW).
• Valid Driver's License and ability to adhere to the Driver Safety Program.
• Ability to provide TB skin test results and required immunizations.
• Basic digital literacy (Microsoft Office and Email).
