What you will do

• Provide high-quality preventative, acute, and chronic care in a fully remote, virtual setting.
• Manage a dedicated panel of patients as their primary care physician.
• Refer patients to specialists or acute care services when clinically indicated.
• Implement population health initiatives, including AWVs and care gap closures.
• Support patients post-discharge to improve treatment understanding and reduce readmissions.
• Adhere to clinical guidelines through continuous self-evaluation.

Requirements

• Active Texas medical license with IMLC compact licensure.
• Board-Certified in Family Medicine.
• 3+ years of post-residency work experience.
• 2+ years of experience in an in-person primary care setting.
• Experience with virtual visits and EPIC EMR.
• Ability to work Monday and Friday with flexible starting hours (7 am-12:30 pm EDT).
