What you will do

• Provide high-quality preventative, acute, and chronic care in a fully-remote, virtual setting.
• Manage a dedicated panel of patients as their primary care physician.
• Execute population health initiatives, including AWVs, care gap closures, and transitions of care.
• Support patients post-discharge to improve treatment plan understanding and reduce readmissions.
• Refer patients to specialists or acute care services when clinically indicated.
• Utilize hirify.global's Clinical AI Platform to reduce administrative burden and improve efficiency.

Requirements

• Active Florida medical license with IMLC compact licensure.
• Board-Certification in Family Medicine.
• 3+ years of post-residency work experience.
• 2+ years of experience in an in-person primary care setting.
• Prior experience with EPIC EMR and virtual visits.
• Ability to work 40 hours per week, specifically including Mondays and Fridays (starting 7 am-12:30 pm EDT).
