What you will do

• Conduct pre-visit preparation and video onboarding for members prior to telehealth visits.
• Manage clinical orders and coordinate DME and skilled nursing services with home health partners.
• Track laboratory testing and imaging results to ensure timely review by clinicians.
• Serve as a member advocate, coordinating care with PCP offices and pharmacies.
• Provide patient education and distribute program materials to improve plan adherence.

Requirements

• MA Certification (active or expired).
• Fully bilingual in English and Spanish.
• Two years of patient-facing experience.
• Must be based in the USA.

Nice to have

• Experience with telehealth services.
• Prior primary care experience.
• Experience helping Medicare patients close stars care gaps and complete preventative screenings.
