What you will do

• Prepare members for telehealth visits through video onboarding and technical assistance.
• Coordinate durable medical equipment (DME) and skilled nursing services with home health partners.
• Manage lab testing and imaging requests, diligently tracking results for clinician review.
• Act as a member advocate, coordinating care across pharmacies, PCP offices, and other healthcare providers.
• Provide patient education and distribute welcome kits and educational materials to members.

Requirements

• MA Certification (active or expired) is required.
• Fully bilingual in English and Spanish.
• Must be based in the USA.
• Two years of patient-facing experience.

Nice to have

• Experience with telehealth platforms.
• Prior primary care experience.
• Experience with Medicare "stars" care gaps and preventative health screenings.
