Salary: $130,000 - $170,000 total compensation (base plus performance bonus).

What you will do

• Perform initial TOC visits including comprehensive medical assessments, medication reconciliation, and ordering necessary labs or imaging.
• Conduct virtual urgent care visits to diagnose and manage acute symptoms and ensure patients receive appropriate care levels.
• Participate in interdisciplinary rounds to identify and manage patients at the highest risk of readmission.
• Collaborate with nurse case managers, clinical guides, and coordinators to provide wrap-around post-discharge care.
• Contribute to the clinical education of the team and foster a safe environment for communication and escalations.
• Utilize the proprietary electronic health information system and provide feedback for interface improvements.

Requirements

• Bilingual proficiency in both Spanish and English.
• Must be based in the USA and available to work MST/PST hours.
• At least 3 years of experience in hospital, post-hospital, or primary care settings.
• Active RN/APRN or Physician Assistant (PA) license in good standing.
• Willingness to obtain licensure in required states within 90 days of hire.
• Active BLS certification at time of hire.

Nice to have

• Experience in internal medicine, urgent care, emergency room, hospital medicine, or geriatrics.
• Experience performing visits via telehealth video platforms.
• Proven track record in managing acute/chronic disease exacerbations such as CHF, COPD, or diabetic emergencies.
