What you will do

• Conduct outreach and patient registration via virtual and in-person channels to engage the Latino community.
• Navigate patients through healthcare logistics, including appointment scheduling, prescription support, and referral coordination.
• Assess for Social Determinants of Health (SDOH) needs and enroll patients in specialized support programs.
• Collaborate with a multidisciplinary care team to triage requests and develop individualized care plans.
• Represent the company at community events to build trust and facilitate group education sessions.
• Provide culturally and linguistically appropriate health education and information in English and Spanish.

Requirements

• Fluency (verbal and written) in English and Spanish.
• Must be authorized to work in the United States.
• Based in Santa Cruz County, CA, with reliable transportation for community visits.
• High school diploma or GED.
• Community Health Worker certification or demonstration of 2,000+ hours of CHW work in the past three years.
• 1-3 years of healthcare navigation and 2-5 years of community organizing or advocacy experience.

Nice to have

• Knowledge of Latino customs and cultural norms.
• Experience with Google Workspace and documentation in EHR systems.
• Training in motivational interviewing.
• Strong existing ties to the local Latino community.
