What you will do

• Deliver individualized, multidisciplinary care to patients with opioid addiction in homes and community settings.
• Assist patients in identifying and seeking treatment for underlying mental and physical health conditions.
• Develop interpersonal, social, and independent living skills using evidence-based interventions.
• Provide education on substance use disorders, symptoms, triggers, and medication adherence.
• Coordinate emergency services including financial, medical, and housing assistance.
• Document patient progress, interventions, and risk factors related to relapse.

Requirements

• Associate of Applied Science in Human Services or Behavioral Health (Required).
• Must have reliable transportation and a valid Driver’s License.
• Experience providing one-on-one or group services focused on rehabilitation and recovery.
• Knowledge of substance abuse populations and treatment planning.
• Strong interpersonal communication and basic computer/word processing skills.

• Bachelor’s Degree in Psychology, Social Work, or other related human service fields.
