What you will do

• Provide high-quality preventative, acute, and chronic care in a fully remote, virtual setting.
• Manage a dedicated panel of patients as their primary care physician to build longitudinal relationships.
• Execute population health initiatives including AWVs, care gaps, and risk capture for risk-attributed patients.
• Support patients immediately post-discharge to improve treatment plan adherence and reduce hospital readmissions.
• Refer patients to specialists or acute care services when clinically indicated.

Requirements

• Board Certification in Family or Internal Medicine.
• Active North Carolina (NC) state medical license (IMLC preferred).
• 3+ years of post-residency work experience and 2+ years in an in-person primary care setting.
• Experience with virtual visits and proficiency with EPIC EMR.
• Availability to work 3x12-hour shifts per week, including Saturday and Sunday.
• Clean background and medical malpractice history.
