What you will do

• Conduct regular face-to-face needs assessments in member homes and nursing facilities.
• Coordinate care across physical, behavioral, and long-term support services.
• Develop and implement individualized care plans to meet specific member goals.
• Facilitate transitions between hospital, home, and community-based care settings.
• Educate members and caregivers on available benefits, resources, and service options.
• Document all case management interventions in the electronic health record system.

Requirements

• Must be based in Delaware with reliable transportation for frequent travel.
• Bachelor's degree in Social Work/Health/Human Services with 3 years of experience OR Master's degree with 1 year of experience.
• Alternatively, current RN or LPN licensure with 2 years of relevant experience.
• Proficiency in PC-based word processing and database documentation.
• Ability to meet regulatory deadlines and comply with HIPAA and privacy standards.
• Dedicated home workspace for business purposes.

Nice to have

• Certified Case Manager (CCM) or relevant social work licensure (LBSW, LMSW, LCSW).
• Experience with Medicare, Medicaid, and managed care environments.
• Background in geriatric special needs, behavioral health, or developmental disabilities.
