What you will do

• Act as a core member of the care team to develop clinical pathways for targeted patient groups.
• Provide immediate assessment, intervention, and support for patients referred by the care team.
• Deliver short-term therapeutic counseling using CBT and Motivational Interviewing techniques.
• Coordinate with family members and support systems to enhance patient functioning.
• Triage and refer patients to specialty mental health and addiction services when necessary.
• Document clinical interventions in the shared medical record for multidisciplinary visibility.

Requirements

• Master’s degree in a Mental Health field required.
• Active licensure as LCSW, LPC, or LMFT required.
• Must be based in Pennsylvania.
• 2 years of experience in evidence-based counseling and diagnosis of mental health disorders.
• Knowledge of the relationship between medical and psychological aspects of health.
• Ability to obtain required state background and child abuse clearances.
